Nursing Enrollment: Requirements
These requirements must be completed between official acceptance and enrollment.

PA State Clearance
Clearances must be issued within six months of the class start date. Please do not complete clearance applications earlier.
Instructions for Obtaining PA State Clearance:
- Navigate to https://epatch.state.pa.us/home
- Click gray colored block Submit a New Record Check
- Read and accept the terms
- Click on Individual Request
- Select Other as the reason for request
- Enter your personal information and confirm
- On the Record Check Request Form, ensure that you list any alias or maiden names
- After entering information, click VIEW QUEUED RECORD CHECK REQUESTS (1) near the bottom of the page
- A Request Queue will show up listing your name, and under it, the option to pay. Click on the gray button that says SUBMIT
- Click on CHECKOUT
- Enter Credit or Debit Card Information to pay the $22.00 processing fee.
- If no record exists, you can immediately print out your clearance certificate:
- Click on the CONTROL # (ex: RI 2345678)
- A Record Check Details page will be displayed. Click Certification Form near the bottom of the page. Your PA State Clearance will appear.
- Download and save your PA State Clearance Form
- Submit your complete PA State Clearance
- Keep a copy for your own records
ACT 33-Child Abuse History Clearance – Instructions
Clearances must be issued within six months of the class start date. Please do not complete clearance applications earlier.
Instructions for Act 33 Clearance:
- Navigate to www.compass.state.pa.us/CWIS
- Click on Create Individual Account (log on if you already have an account)
- Select NEXT to go to the Create Keystone ID: Profile Information page
- Create a Keystone ID, fill in personal information, and set up Security Questions
- Retrieve the temporary password from your email
- Go back to www.compass.state.pa.us/CWIS and Log On
- Click on INDIVIDUAL LOGIN
- Click on ACCESS MY CLEARANCES
- Enter your newly created Keystone ID and Temporary Password, then click Log In
- Follow security prompts, then change your password
- Enter your Keystone ID and newly created Password and follow security prompts
- Click on CREATE CLEARANCE APPLICATION
- When asked, enter EMPLOYEE OF CHILDCARE SERVICES for Application Purpose
- Complete Application. You will need to provide:
o Any aliases or prior names
o Addresses you have lived at since 1975 (as much information as you remember)
o Name and current age of any persons you have lived with since 1975. You must include a parent or guardian name in the list.
- Type your name to sign the document. You do not have an organization code.
- Use Credit or Debit Card to make $13.00 payment
- In approximately 1 week you will receive an email that your clearance is ready to print. Use
Keystone ID and Password to log back in and ACCESS YOUR CLEARANCE
- Download and save your completed Act 33 Clearance.
- Submit your completed Act 33 Clearance to PTC. Save a copy for your own records.
FBI Criminal Background Check (ACT 73) Instructions
Clearances must be issued within six months of the class start date. Please do not complete clearance applications earlier.
- Register online: https://uenroll.identago.com/
- Click on Digital Fingerprinting
- Enter Service Code 1KG756 in the middle of the page and then select GO
- Click Schedule or Manage Appointment and fill in information.
- Select Name/Method of Contact if it is not already selected at the top of your page.
- Then fill in the following information:
- Essential Info
- Employer (Student Name and Address)
- Citizenship
- Personal Questions
- Address
- Documents (Select a document that you will be able to take with you when you go for your fingerprinting appt. We suggest Drivers License)
- Location (type in zip code, hit search, select location and then select Next) This will be the location you will go to have your fingerprints done.
- Choose Date & Time
- Review all information and click Confirm.
- The cost is $25.25. You will be required to pay this fee at the location of your choice when you have your fingerprints done.
- They accept: Money Orders or Credit/Debit Card.
- In 2 to 4 weeks, you will receive your FBI Clearance by postal mail.
- Scan/Email a copy to your Enrollment Coordinator OR upload to your PTC Dashboard
CPR Certification
Clearances must be issued within six months of the class start date. Please do not complete clearance applications earlier.
American Heart Association (AHA)
PTC only accepts CPR Certifications issued directly from: American Heart Association. We do not accept CPR Certifications from any other company (even if the card says it follows the rules of the AHA). Companies not accepted include, but are not limited to: Health Safety, American Red Cross, Pro First Aid Advanced, National CPR Foundation, American Academy of CPR & First Aid, and American Health Care Academy.
Paying for and becoming CPR certified through the American Heart Association (AHA) prior to the class start date is the responsibility of the student.
Directions to Register for Classroom Instruction:
- Navigate to www.heart.org
- Click on CPR
- Click on Find a Training Center
- Enter your location
- Click on Filter your Search
- Course Style: Classroom
- Course Name: BLS Provider
- Language: English
- Date Range: Choose dates you are available between.
- Once you find a location convenient for you, click DETAILS and RESERVE.
- If a location has specific instructions under DETAILS, please follow them accordingly.
Once scheduled you will receive a Confirmation Email with Date, Time, Location and Instructor contact information. Payment information will come from the instructor.
Submit completed CPR Certification to PTC.
Directions to Register for Hybrid Online/Classroom Instruction:
- Navigate to www.heart.org
- Click on CPR
- Click on Find a Course
- In the Finding Right Course block choose:
- Audience: Healthcare Professionals
- Topic: Basic Life Support (BLS)
- Course Format: Online + Classroom
- Click Basic Life Support (BLS) Course/American Heart Association CPR & First Aid
- On Basic Life Support (BLS) Course Options Page, scroll to Blended Learning block
- Click Online Portion
- On Heart Code BLS Page click Make Purchase, then add to cart and pay.
- Complete Online Portion
Once the online portion is completed, schedule your Hands On Skills Session by following same directions until Step 6. Then:
- Under Blended Learning block click on Hands on Skills Session.
- Choose location and date or you can contact the Instructor with Phone Number and Email provided on the site. For the Hands On Skills Session, you will be billed by the instructor.
- Once scheduled, you will receive a Confirmation Email with Date, Time, Location, and Instructor contact information. The confirmation explains that Part #1 Online Portion must be completed prior to attending a Hands On Skills Session and you must show proof of completion at the beginning of the class with a receipt on your phone or printed from a computer.
Submit completed CPR Certification to PTC.
